I give my permission for any photographs, videotapes, sound recordings, etc. taken of my child during a rehearsal or performance of a DPAC class to be used for promotional purposes.  This includes, but is not limited to, newspapers, brochures, flyers, video, television, website, etc.

I give my permission for a director or staff member of DPAC to authorize medical treatment for my child in case of an emergency in my absence and if every effort has been made to contact me.

 

I hereby indemnify and hold harmless DPAC, South Sanpete County School District, Snow College, Manti City, its representatives, employees, directors, and officers from any and all cost, expense, claim, or damage of any kind, arising from or relating to my child’s participation in our program.
